Complete the Access Request Form or call the NDIA directly on 1800 800 110 to begin your application.

A Local Area Coordinator (LAC) or NDIA planner will work with you to explore your goals and needs.

Step 4: Receive Your NDIS Plan

Once approved, your NDIS plan will be delivered to you—either in person or by mail.

Step 5: Start Accessing Support

You can choose how you want to manage your funding—on your own or through trusted providers like us.

Step 6: Choose a Support Coordinator (Optional)

You can engage a support coordinator or your LAC to help you implement your plan—or simply let Activity Care Services help you navigate it with ease.
